Converting CSV log to ADIF

Greetings, can anyone tell how to convert CSV log from SOTA data base to ADIF. for input into my log. I have tried log4om but it can only merge into an existing log, which I do not have as yet. I am trying to establish a log using the downloaded SOTA log.

Any advice, guidance or help will be much appreciated.
